ASU UL Course Application

Instructions 

Students wishing to take ASU Universal Learner courses are required to complete the application below for each course enrollment. Please let your PEC know that you would like to take an ASU ULC course. Applications must be completed 2 weeks prior to the end of the enrollment window to allow time for review, approval, & processing of the ASU ULC Application. 

 

Upon approval, students will receive an email with the Acknowledgement and Acceptance of ASU ULC Terms for electronic signature. Once the form has been initialed and signed by both the student and parent/guardian, students will be added to our ASU portal. Students will then receive login information for ASU’s student portal and a registration packet with instructions and additional information.

Please list any & all high school courses that would be pre-requisites to this particular ASU UL course (i.e., any courses that would be relevant to this particular course)—you must also list the school where this course was taken, your instructor, & your final grade in this course:

Please provide a short (minimum 200 words) essay that answers and includes:

  1. Use your best grammar, spelling, research techniques (documentation), and writing skills, as they will be assessed in this essay.
  2. Why you would like to take an ASU UL course? (Please be very specific and explain why you would like to take this ASU UL course.)
  3. What are your goals for taking an ASU UL course?
  4. How do you think this ASU UL course will further your educational goals?
  5. Are you planning on taking the ASU UL course for university credit? Why or why not? (Note: You are NOT required to take the course for university credit; you may take the course for HS credit only, if you prefer.)
  6. List 3 references (preferably CMASAS teachers/instructors in the subject area in which you would like to take an ASU UL course—i.e., math, English, science, etc.) that we could contact to discuss your academic abilities to take & succeed in an ASU UL course. Please include the reference name, email address, and phone number.
